Receptionist was rude, with an attitude. I am obviously asking questions to get an understanding of the treatments and she’s answering as if I were stupid. Learn better customer service skills . Can’t remember the workers name. Either way you are just a worker there and your job is to give out information. Don’t understand the attitude answering questions as if I were stupid and that is your job to begin with.

Beautiful office and location! The staff is kind. Everyone is friendly and you can tell everybody loves their job. The front desk team welcomed us warmly. In particular Marilu helped answer all of our questions regarding NAD. We will stop by soon for NAD and B12 injections. Wished I would have found Formula Wellness sooner. :)

I always get really great service and everyone is very pleasant. Most recently I worked with Sabrina and she was very patient and thorough. Normally, I work with Taylor who I highly recommend for laser hair.

Hands down the best wellness experience I’ve ever had! My new patient package appointment exceeded every expectation. I came in a couple of weeks ago for my very first Hydrafacial with Courtney, and let me tell you — the experience was absolutely phenomenal! Courtney was incredibly kind, attentive, and took the time to explain everything in immaculate detail, which made me feel completely at ease throughout the entire appointment. The office staff were just as wonderful — warm, welcoming, and professional. I want to give a special shoutout to Marilu, who is truly a sweet and kind soul. She made the whole process smooth and was so helpful in answering all my questions. I genuinely can’t wait to return for my next facial. Highly recommend this place if you’re looking for top-notch service and care!

Courtney at Formula Wellness is truly amazing!! Her product recommendations have made such a difference in my skin, it’s been healing so much since I started following her advice. She really listens, explains everything clearly, and tailors recommendations to what your skin actually needs. I trust her completely and couldn’t be happier with my results!!

No longer providing the special medical and wellness services it used to do. I have been a happy patient at FW for almost 10 years and pay a couple of hundred dollars per month. That used to provide direct access to a provider but until a few weeks ago that changed. You're not even allowed to email the doctor (unless you pay more). So much for customer loyalty. Now it's more akin to a regular doctor's office with central, indirect access. With several nurse practitioners having left the office, it has also been difficult getting simple appointments.

I was recommended formula wellness by their employee Sabrina who made the process of setting up my consultation seamless. Courtney gave me an easy to follow routine for before and after my chemical peel along with explaining the how and why behind each step. I left their office feeling very confident and today. If I could give 10 stars I would!

I had such a wonderful experience at Formula Wellness with Sabrina, who was my nurse. She took my blood and administered my IV while explaining everything in a calm and reassuring way, which made me feel so comfortable. She was incredibly kind and caring and truly helped me feel better during my visit.
